The alternative to ovarian stimulation is an operation called laparoscopic ovarian diathermy (LOD), also known as "Ovarian Drilling". Laparoscopic ovarian drilling is considered an appropriate option for patients who have failed oral medications and are unwilling or unable to use gonadotropins, or those patients already undergoing laparoscopy for another indication.
Ovarian drilling is a laser fiber or electrosurgical needle, which is used to puncture the ovary 10-12 times to reduce the androgenic levels. The studies have shown that about 80% will benefit from this treatment
Procedure
This procedure is done under short general anesthesia. Patient can leave the hospital on the same day of surgery. Ovarian drilling is usually done through a laparoscope (small incision). The surgeon makes a small cut (incision) in the abdomen at the belly button. The surgeon then places a tube to inflate the abdomen with a small amount of carbon dioxide gas so that he or she can insert the viewing instrument (laparoscope) without damage to the internal organs. The surgeon looks through the laparoscope at the internal organs. Surgical instruments may be inserted through the same incision or other small incisions in the pelvic area. The ovaries are identified and several small holes made in each ovary, either with a fine hot diathermy probe or laser.
Because the incisions are so small, laparoscopy is often called "Band-Aid surgery."
Side effects are rare, but may result in adhesion formation or ovarian failure if there are complications during the procedure.
By 12 months after LOD the average pregnancy rate is around 60% to 80%, the greatest success rates being in women with a shorter length of infertility (less than 3 years) and a higher level of the hormone LH (>10 iu/l). Advantages of LOD include the fact that it may improve other symptoms of PCOS, such as menstrual disturbance, as well as avoiding the need for stimulatory drugs and their increased risk of over-stimulation and a multiple pregnancy.
